Nominators should fill out the online form or print out the paper form (PDF). Alternatively, you may submit the cover sheet electronically and submit any supporting materials via fax or surface mail. The answers to all questions must be typed on separate pages and attached to the cover sheet. The package may include supporting materials such as newspaper clippings, letters or other documents. The entire nomination, including the cover sheet should not exceed 10 single-sided pages and must be easily reproduced in an 8 ½ x 11" format. If you submit the cover sheet electronically, please clearly label any supporting materials when you submit them via fax or surface mail. All nomination materials become the property of AFT and cannot be returned.

Nominations may be submitted electronically, by surface mail or fax. They are due at AFT's national headquarters by 5 PM EST on Monday, December 10, 2007.
